New to Lifestyle Medicine?

Lifestyle Medicine is a speciality clinic that compliments your general cardiology care. Like a typical cardiology visit, Lifestyle Medicine appointments are also billed to your insurance company. You will pay your standard/applicable copays for each visit. There is no additional fee to participate in our Lifestyle Medicine program. Insurance covers both individual and group appointments.

The 1st visit will last approximately 45 minutes. There will be a brief questionnaire to fill out, which will be sent to you prior to your appointment, or you can access it here.  During your first visit, we will discuss your health concerns and goals to determine the role of Lifestyle Medicine in your care.

Primary delivery of Lifestyle Medicine appointments is through Shared Medical Appointments following your initial consultation. We work together closely for a year, meeting once-weekly for 6 weeks. Following the 6 week sessions, we will continue with 3-month, 6-month, and 9-month follow-up sessions.

Classes are available on Tuesday and Thursday, and last 2 hours (9:00-11:00 am and 1:00-3:00 pm). Choose the day and time that works best for you.
